Instruction Page

I. WHAT THE JONATHAN A. MINKOFF A CAPPELLA AWARD IS
Grant monies are awarded on an objective and non-discriminatory basis to individuals and groups with proposals to foster and promote the art form of a cappella and benefit the community.

II. WHAT TO SEND
In one single package send us the following:

(A) THE AGREEMENT
Completely read and fill out the 1 page Agreement. This is a legal document and it is important that you fully understand it before signing. If you are a minor you will also need your parent or guardian to sign.

(B) YOUR PROPOSAL
On separate typed/printed pages, with your name on each page, include:

1. EXPENDITURE LIST. Include the purpose and amounts (including a total) of all funds you are requesting as well as a time frame of 1 year or less for their expenditure. You may provide this in chart or list form.
2. ESSAY 1. Describe how this grant will foster and promote the art form of a cappella music and benefit the community.
3. ESSAY 2. Describe your past experience in a cappella, focusing on how you have worked to foster and promote the art form of a cappella music and benefit the community. You may do this in essay or resume form or as a combination.
4. (Optional) Provide copies of any materials you feel would support this application: recordings, letters of recommendation, price quotes, etc. (Materials will not be returned.) Include a list of the attached materials.

III. WHERE TO SEND YOUR COMPLETED APPLICATION

You will receive a call or email confirming our receipt of your application materials. You may be interviewed or asked for additional materials.

IV. WHEN TO SEND YOUR COMPLETED APPLICATION
Materials are reviewed on a rolling basis, but decisions are usually made each year in February and March, just before they are announced at the East Coast A cappella Summit. For this reason, we recommend that you ensure that we have received your complete application by February 1st.

V. “WHO” MAY APPLY
US students, teachers, amateurs, professionals, and organizations, including a cappella groups may apply for this grant. (see the attached Agreement for additional restrictions).

VI. HOW MANY GRANTS ARE AWARDED AND HOW LARGE IS EACH GRANT?
At least one grant will be awarded each year. Additional grants may also be awarded. Amounts are based on the needs presented in the applications. Grants are expected to total approximately 5,000 per year.

Do you plan to attend the East Coast A cappella Summit? __YES __NO www.casa.org/eastcoastsummit This is where the award will be presented. You will still be considered for the award, even if you cannot attend.

This document is a binding legal agreement between The Contemporary A cappella Society of America (“CASA”) and the applicant for the Jonathan A. Minkoff A cappella Award (“You”). In consideration of having your application materials reviewed and of any funds or other benefits which may be awarded to you, you, personally and on behalf of your organization agree, warrant and affirm under penalty of perjury that:

1. You are a citizen of the United States, or your organization is based in the United States and you have not been convicted of a felony. You are not a “supporting organization” (509(a)(3) public charity) or any entity prohibited from receiving funds under any law.

2. You are not: a member of CASA’s current Board of Directors, the Award Committee, the immediate families of anyone on CASA’s current Board of Directors, Jonathan A. Minkoff, a Successor Founder or the Award Committee, nor are you any organization in which any of the aforementioned entities own a for-profit financial interest (collectively hereinafter referred to as “Prohibited Persons”).

3. You will expend grant monies only for the purposes and in the manner stated in your proposal. You will not expend grant monies to influence public elections or to lobby or to benefit “Prohibited Persons” or any organization in which any Prohibited Person has a for-profit financial interest.

4. In the event a particular expenditure as described in your proposal becomes impossible or impractical, you will contact the Award Committee to either A) receive permission in writing for an appropriate substitution expenditure, or B) immediately return to CASA the unexpended portion of the funds.

5. You will expend all grant funds within one calendar year of receiving the funds or forfeit the remainder and return it to CASA immediately.

6. You will maintain a regularly updated, free or paid membership account which CASA may use to contact you, with CASA’s online service at www.casa.org for 5 years from the date of this application.

7. You will maintain detailed records, including receipts, detailing how the grant monies have been expended and you agree to provide them to CASA as requested.

8. You will credit “The Jonathan A. Minkoff A cappella Award and CASA, The Contemporary A cappella Society of America” verbally, electronically and in print in connection with each and every use of the funds. E.g. “This recording/website/microphone/performance, etc. was provided by a grant from the Jonathan A. Minkoff A cappella Award and CASA, The Contemporary A cappella Society of America.”

9. You will be interviewed, make reasonable public appearances as requested, and allow your name, image, likeness, application materials and personal story to be used without limitation by this award and by CASA and by such others as they authorize in perpetuity throughout the Universe without payment to you.

10. You will avoid actions or statements which would reflect negatively on the reputation of this award or CASA and failure to do so may result in forfeiture of all monies received in addition to other remedies.
